Supportive Living in Chicago and Surrounding Areas - Financial Assistance / Financial Aid
Concord Place has been selected by the Illinois Department of Aging as one
of the few Senior Care Facilities to be included in the State of Illinois Supportive
Living Program. This program was developed to provide lower-income seniors and
seniors with disabilities as an alternative to nursing home care. Assisted Living,
or as the State of Illinois calls it Supportive Living, is now an option for
all seniors, not just the wealthy.
Depending on your financial position and health status, if you qualify Medicaid
will reimburse Concord Place for the cost of Assisted Living care. The program
combines apartment style living with personal care and other services. You can
live independently; take part in your own decisions while receiving the personal
care and attention you require.
To apply for assistance with the Illinois Depart of Public Aid you will need the following information before moving into Concord Place:
- Last three (3) years of savings account statements
- Last three (3) years of checking account statements
- An explanation of deposits and withdrawals from either of the above if over $500, excluding Social Security income
- Medicare Card
- Health Insurance Cards
- Proof of pension income or other income
- Health and life insurance policies with a statement of cash value of life insurance and cost of health insurance
- Any prepaid burial plans with date of purchase and cash value
- Stocks, bonds, other securities, and safe deposit receipts
- Deed and tax statements for property owned currently or sold in the last three (3) years
- Verification of any accounts closed in the last three years
- If you have supplemental insurance, we will need a copy of your insurance card along with a premium statement
- Birth certificate or baptismal record
Concord Place has in-house staff that will review all your documents and file all the necessary paperwork. Because we use our own, in-house staff we are able to file paperwork faster than any other facility. The majority of facilities outsource their paperwork; we never do!
